A Clinical Study of Suppurative Arthritis



Abstract
A clinical analysis of the suppurative arthritis was made on 53 patients who were treated from May 1975 to October 1981.
Results were obtained as follows:
1. Among the 53 patients, 39 were male and 14 were female. The most prevalent age was 7 to 10 years(17 cases, 32. 1%).
2. Lag period to treatment in most cases were less than 5 days (23 cases, 43.4%).
3. The most common symptom and sign was pain and tenderness on the affected joint (38 cases, 71.1%). 4. The most common underlying cause was minor trauma (20 cases, 37.7%).
5. The most commonly affected joint was the hip (24 cases, 45.3%), and next common was the knee joint (21 cases, 39.6%) .
6. The most common causative microorganism was staphylococcus aureus (24 cases, 68.6%) and penicilline resistance rate was 79.2%. Mixed infection was observed in 3 cases.
7. The late sequelae was found in 10 cases; permanent dislocation in 1, chronic osteomyelitis of the femur in 2, spontaneous joint fusion in 3, limited joint motion in 4.
8. The basic management was early arthrotomy with drainage (13 cases, 24.5%) and continuous irrigation (24 cases, 45.2%). In conclusion, the successful treatment of suppurative arthritis had been required to put the joint at rest, administer parenteral antibiotics, and lavage the joint by arthrotomy.
